Locating a participating provider in the PHCS network begins with the specific network logo on the front of your medical ID card. Please locate the PHCS logo on your card and follow the corresponding instructions below.

  • After acknowledging you have read the disclaimer at the bottom of the screen, click on the green “Select Network” button
  • When selecting your network, choose “PHCS,” then “Preventive Services Only”
  • Enter one of the search criteria suggested in the search box to begin your search
  • If your browser settings don’t allow your location to be detected, enter a zip code

Anthem Blue Cross Vision

You have many choices when it comes to using your benefits. As a Blue View Vision plan member, you have access to one of the nation’s largest vision networks. You may choose from many private practice doctors, local optical stores, and national retail stores including LensCrafters®, Target Optical®, Sears Optical® and JCPenney® Optical. You may also use your in-network benefits to order eyewear online at Glasses.com and ContactsDirect.com.

To locate a participating network eye care doctor or location, log in at anthem.com/ca or from the home page menu under Care, select Find a Doctor. You may also call member services for assistance at 1-866-723-0515

Out-of-Network – If you choose to, you may instead receive covered benefits outside of the Blue View Vision network. Just pay in full at the time of service, obtain an itemized receipt, and file a claim for reimbursement up to your maximum out-of-network allowance.

Anthem Blue Cross Dental

To select a dentist by name or location:

Call the toll-free customer service number listed on the back of your ID card.

Employer Mandate 2020

Employers must offer health insurance that is affordable and provides minimum value to 95% of their full-time employees and their children up to age 26, or be subject to penalties. The mandate applies to employers with 50* or more full-time employees, and/or full-time equivalents (FTEs) who work 30+ hours per week. Employers are required to offer coverage that provides “minimum value” and is “affordable,” or be subject to penalties.

EXEMPTIONS FOR 2019 AND BEYOND

  • For the 2019 plan year, the Shared Responsibility Payment (“mandate,” “penalty”) no longer applies. You don’t need an exemption for 2019 or beyond.

Find health coverage exemptions

  • Most people who don’t have qualifying health coverage must either pay a fee or qualify for an “exemption.”
  • Health coverage exemptions are available for a variety of reasons: certain life events, health coverage or financial status, group membership, and more. If you qualify, you don’t have to pay the penalty for the months you were uncovered and the exemption applies.
